About the role
Ballina Community Health is seeking a Health Clinician to join their Aged Care Assessment team to provide comprehensive and appropriate assessment and care planning for Aged Care clients in line with Northern NSW Aged Care Assessment Team guidelines.
If you have:
- Relevant qualification in Social Work, Psychology, Physiotherapy, Occupational Therapy or Registered Nurse prior to commencing and registration with the Australian Health Practitioner Regulation Agency (AHPRA) where required or eligibility for membership of the appropriate professional body. To be appointed as a Level 2 Health Professional evidence must be provided of three years' clinical experience
- Ability to utilise problem solving skills and a multidisciplinary team approach in the planning, delivery and coordination of patient care and effectively prioritise and organise own work/caseload
- Effective communication through the use of information technology, written, verbal and interpersonal skills with the ability to identify key messages, issues and concerns when communicating with others
- Knowledge and understanding of person centred care, quality improvement and evidence based practice
