The Role
Are you a Nurse Practitioner with a passion for and experience in cardiovascular health? Are you looking for a role where you can make a difference in the lives with people diagnosed with Heart Failure?
Our team is seeking an experienced Nurse Practitioner to co-ordinate our Heart Failure Clinic.
The key purpose of this role is to provide high quality, patient centred care that improves the quality of our patients live and prevent patient readmission to hospital. This is achieved by providing access to a specialised Heart Failure Clinical team, supporting the construction and monitoring of personalised treatment plans, and providing continuous support and Heart Failure education to our patients.
This position will also liaise with referring GP’s and participate in educational events promoting the program.
This is a part-time 0.4FTE role, with the view to increase hours depending on patient demand. It requires you to be self-driven with regular reporting to the General Manager and head of the Heart Failure team.
The Ideal Candidate
- Be appropriately registered with AHPRA (essential).
- Have minimum 4 years’ experience in cardiac nursing.
- Be able to demonstrate current experience in clinical education.
- Hold a Masters qualification in nursing.
- Have excellent written, verbal & presentation skills.
- Have a contemporary knowledge of best practice in nursing care.
- Be highly accountable and demonstrate a strong, supportive and engaging communication style.
- Be passionate about patient care and achieving high quality clinical outcomes.
- Be a team player working with working with cardiologists, administrative and clinical teams equally.
The Environment
You will be required to work at our Mount and Murdoch locations, with the view to provide face to face clinics and or telehealth support to patients. You will be required to liaise directly with cardiologists, clinical and administrative staff.
