Key Responsibilities:
- Provide high-quality, personalised support to clients with complex physical and mental health needs.
- Assist with activities of daily living including personal care, medication management, and meal preparation.
- Implement and follow individual care plans to ensure clients' safety, dignity, and well-being.
- Foster a supportive and inclusive environment, encouraging clients to achieve their personal goals.
- Collaborate with healthcare professionals, families, and other Support Workers to deliver comprehensive care.
- Monitor and report on client's progress, documenting changes and updating care plans as necessary.
Requirements:
- Previous experience in a high-intensity support role or a related field.
- Relevant qualifications in healthcare or a related discipline (Certificate III in Disability, Mental Health, or similar).
- Excellent communication and interpersonal skills.
- Ability to work flexible hours, including weekends and nights.
- Strong problem-solving skills and the ability to remain calm under pressure.
- A valid driver's licence and access to a reliable vehicle with comprehensive insurance.
- National police clearance, NDIS screening check, Working with children check, and a current First aid and CPR certification.
