- $54.63 hourly base + salary packaging + super (pro-rata for part time)

About Hospital in the Home & Virtual Care
The Hospital in the Home program facilitates the shift of traditional acute care to the patient’s own home without compromising patient care. This care may take place via in-home nursing and support services, via video telehealth or via admission to the Hospital in the Home – Virtual ward, where higher acuity care can be provided in part through the utilisation of wearable technology.
About the role
The Hospital in the Home (HITH) Case manager will proactively manage the transfer of patients from a traditional inpatient model to receiving high quality, acute care in the patient’s own home. They will manage the needs of patients in a collaborative fashion to ensure high patient satisfaction, while facilitating hospital flow and safe and effective discharge planning to community services post HITH admission. They will provide clinical expertise and support to the services (both internal and external) providing care to the patients in a community setting. They support the provision of care by the most effective means – be that Telehealth, virtual ward admission (including the use of wearable monitoring devices), or other Appropriate care pathway in response to acuity and the needs of Austin consumers
A regular day will see you:
- Assess referrals to Hospital in the Home, ensuring safe, high quality care is arranged for an inpatient at home
- Support nurses attending to care in the community, providing a point of escalation and responding remotely to deterioration
- Assess clinical information remotely via wearable technology, engage in virtual ward rounds & video telehealth reviews
- Support comprehensive discharge planning back to community services

To be successful in this role you'll bring with you:
- Highly experience Registered nurse, who has excellent assessment skills and clinical judgement
- Flexible problem solver, open to new ways of providing clinical care
- Confident with learning new technology and ustilising digital services in the provision of care
- Post graduate in critical care/cardiac nursing, or other related post grad qualification (desirable))
- Excellent communication skills
