Based in Salisbury South, as our invaluable Home Care Coordinator, you will be responsible for delivering exceptional customer service to clients, families and advocates, and coordinating services tailored to suit the individual clients needs. This includes liaising with external agencies, conducting effective assessments and reviews of client needs, planning, and coordination and case management.  

You’ll also make an impact by:

• Developing and planning individualised, high quality services that reflect the needs of the client
• Monitoring and reviewing client needs and update Agreed Support Plans to reflect any changes
• Ensuring services are client focused and based on dignity, respect, cultural sensitivity and informed choice

What you’ll bring 

To succeed in this role, you will have:

• Certificate III in Community Services or equivalent work experience
• A strong theoretical approach to working with ageing population and demonstrated practical application 
• An understanding of community resources and supports as well as excellent verbal and written communication skills
• Strong interpersonal, communication and customer service skills
• Effective oral and written communication skills
• Computer literacy in a Windows environment and good knowledge of the Microsoft Office suite
• Current driving licence and a reliable motor vehicle
• A valid NDIS Worker Screening clearance or be willing to obtain one. 

You’ll stand out from the crowd if you have some of the following:

• Experience in the aged care industry, familiarity with aged care standards.
