Multicultural Care is looking to recruit an experienced ClinicalAged Care Advisor to work within our team in coordinating and growing Multicultural Care’s Home Care Packaging program (HCP). We are looking to recruit someone for this position immediately to start working with us in our new offices in Burwood!

The Role
The Clinical Care Advisor position is responsible for the provision of high-quality case management services for language-specific clients across programs by undertaking a comprehensive assessment of each client in collaboration with the client and their family or carer, determining a number of health and well-being maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain a quality of life.
This Includes
- The position will be responsible for the delivery of Client Care and Clinical Care to MC clients.
- Monitored the clients progress to ensure optimal outcomes are achieved. If the needs of the organisation and client groups change.
- The flexibility to take on or drop other projects as directed by the Team Leader.
- Maintaining client assessment and Care Plans and progress notes.
- Making regular visits to existing and new clients.
- Ensuring the provision of these services are always in line with the funding bodies' requirements, Social Justice and Access & Equity principles, WHS requirements and the organisation’s philosophy, mission, policies and procedures.
About You
This is a crucial role with a high-level community partnering through frequent engagement, communication, and evaluation of programs. To be successful in this role you will have:
- Degree in Allied health is required.
- Registration with AHPRA is essential.
- Demonstrated experience in Case Management.
- Experience working in the Aged Care or disability sector.
- Demonstrated understanding of needs and issues of people with disability from CALD backgrounds.
- Demonstrated experience in needs assessment and management of the elderly.
- Demonstrated ability to supervise and develop out of office staff and speak a second language.
- Understanding of work health safety in home and community settings.
- High level of communication and ability to work collaboratively is essential.
- Experience in Proda, MAC & Alayacare is highly regarded.
