We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a crucial role in coordinating the care of our Package Clients, providing them with the support they need to lead fulfilling lives.
Key Responsibilities:
- Coordinate and manage the care plans for our Package Clients to ensure their individual needs are met.
- Work closely with healthcare professionals, caregivers, and family members to develop comprehensive care strategies.
- Conduct regular assessments to monitor the well-being of clients and make necessary adjustments to care plans.
- Foster strong relationships with clients, promoting a sense of trust, respect, and dignity.
- Utilize excellent communication skills to liaise with internal teams, external stakeholders, and clients' support networks.
Qualifications:
- Tertiary qualification in Health, Disability and or related profession and or previous experience in needs assessment, care planning and/or case management.
- Well-developed interpersonal skills and the ability to discuss and resolve issues and promote the services.
- Advanced computer skills, including the ability to use Microsoft Office, email and web based applications.
- Knowledge of Home and Community Care programs.
- Current satisfactory Police Check
- Current valid Driver’s License Desirable:
- Ability to network and liaise within the aged care and disability sectors including negotiation and conflict resolution skills
- Previous experience with a client management software program
- Familiarity with the Home Care Common Standards.
