The Village Glen, one of the largest and most well-appointed retirement communities on the Mornington Peninsula, is seeking a Home Support Partner – STRC Program to join our team working in our Short-Term Restorative Care (STRC) Program. The STRC program provides community-based services to frail older people who have complex care needs and live in the STRC program catchment – Southern region.
In this permanent part-time position, you will be responsible for the provision and co-ordination of quality care that meets the individual needs of the client and the program standards and guidelines.

Your responsibilities will include working with a team of allied health professionals and form part of the multi-disciplinary team working in the STRC program across the broader Southern Metropolitan region. Our clients are over 65 years old, living in their own homes, which may be within the village or across the broader Mornington Peninsula/Southern region. This dynamic, varied role will keep you engaged, providing job satisfaction along the way.
Key Responsibilities:
- Undertake care planning in line with Flexicare Policy and Procedures and STRC program requirements for MDT meetings.
- Follow up incident reports submitted regarding clients
- Keep progress notes on all interactions with clients and their representatives, service providers etc - document in accordance with legal requirements and the organization’s Policies & Procedures
- Work with clients to manage their care budgets
- Participate in quality, WHS and team meetings.
- Participate in Quality Improvement activities
- Escalate Clinical concerns to STRC lead.
- Develop strong networks and relationships with the targeted group’s communities
Required Skills and Qualifications:
- Qualification in Community service with case management, Social Work, Allied Health, RN / EN (preferred).
- A demonstrated ability to work within a human service organisation including administration, and financial management skills
- Excellent written and verbal communication skills
- A strong commitment to working with frail aged people
- A motor vehicle driving licence.
