Reports to:Home Care Manager
Role Overview: As a Home Care Partner, you'll play a vital role in assisting clients across Sydney and surrounding areas in navigating Home Care (HCP) and Linkages Commonwealth Home Support Program (CHSP) Packages, enabling them to maintain independence within their homes for longer. Your responsibilities will include case management, care coordination, and effective brokerage of funds, ensuring the delivery of person-centered care plans.
Key Responsibilities:
- Develop and implement client-centered care plans, aligned with individual goals.
- Coordinate referrals and liaise with support service providers to meet client needs.
- Facilitate regular reviews and documentation of care plans in collaboration with clients and carers.
- Organize and lead case conferences for goal reviews, ensuring effective communication with all stakeholders.
- Maintain accurate records in Alayacare and ensure ongoing client file maintenance.
- Tertiary qualifications in Nursing, Social Work, or a Health/Welfare discipline (Essential).
- Registration with ASWA or relevant professional body (Desirable).
- Experience in case management for older individuals or people with disabilities, demonstrating a deep understanding of their needs and challenges (Essential).
- Strong knowledge of relevant legislation impacting service delivery and practice.
