This role will lead an effective and high functioning team of Care Coordinators and their staff to deliver Home Care services to our valued clients.
Key to success will be ensuring that service delivery targets are achieved, funding body requirements are met, and care and case management to Salvos Home Care consumers is coordinated and delivered as required.
About you
- Tertiary degree-level qualification in aged care, allied health, nursing or related field, with a minimum of two years' professional work experience
- Minimum three years' work experience within Aged Care, preferably including Home Care experience, with proven ability to deliver and coordinate client-directed services within applicable compliance frameworks
- Demonstrated effectiveness in leading and managing a team of staff in an aged care, health or community services setting to achieve best practice outcomes.
- Strong interpersonal communication skills with demonstrated ability to develop and maintain effective professional internal and external stakeholder relationships and manage a human services business
- Demonstrated high-level written communication skills and computer literacy
- Demonstrated understanding of and ability to support and model TSA mission, values and ethos
- Minimum 3 years experience in a management role
- Demonstrated experience in Home Care setting - Home Care Packages
- Demonstrated experience in coaching and mentoring staff
- Demonstrated organisational and time management
- Ability to form strong partnerships with internal and external stakeholders
- Demonstrated excellent customer service skills
- Calm and considered disposition
- Monday to Friday 9am to 5pm with some on-call work when required
