About the role
We have an opportunity for a Home Care Senior Support Worker to join our growing Home Care Team (Community Aged Care). This position is being offered on a permanent part time basis, based in the South Suburbs (Mandurah, Rockingham) from Monday to Friday. You will enjoy leading, mentoring and guiding your own team to support our clients to live independently in the community. You will also provide direct support to older people in the community promoting independence, facilitating connection with others, and working in partnership to meet individual goals.
About you
As a strong leader you will work to promote a supportive team environment and coach others to deliver service to our community members, you will also have:
- Certificate III in Aged Care or Community Work or equivalent(essential) and working towards or completion of Cert IV (desirable)
- Ability to work across Monday to Friday
- Experience working in Aged Care and/or Disability sector and supporting clients with complex needs
- Basic administration skills
- Accurate data entry and case note writing
- Demonstrated understanding of Person-Centred Care and wellness/reablement approaches
- Current C Class Driver’s Licence
- COVID and Influenza vaccinations are preferred but not required
- National Police Clearance (obtained within the last six months)
