Amazing Procare is a rapidly expanding Aged Care and NDIS service provider based on the Central Coast and Hunter Region in NSW and across the Tweed Coast, Gold Coast and Brisbane South areas in QLD. We are currently seeking a Support Coordinator/Psychosocial Recovery Coach, reporting to the Regional Manager you will carry a caseload across Tweed Coast, Gold Coast and Brisbane South areas. Based out of our Coolangatta Office with travel across the region when required.
You will also be offering psychosocial Recovery Coaching for those in our community who require assistance navigating services and supports approved by their NDIS plan.

In addition to this, the right applicant must have a passion for quality service provision, be supportive and respectful. We will provide training in our comprehensive client management system.
Your skills and knowledge will be valued and utilised on a daily basis. You will be required to carry out Coordination as a part of the role. As a Support Coordinator, you will use your skills and experience in unique ways to promote empowerment, choice and control, and community inclusion for the people we support.
You will focus on addressing barriers and reducing complexities in a supportive environment using a strengths-based approach.
You will be provided with ongoing training and mentoring through team meetings & manager collaborations to ensure you continue to progress through your career.
Duties and responsibilities:- Supporting Participants to achieve their Goals in line with their plan
- Psychosocial Recovery Coaching
- Liaising with informal supports
- Working alongside a number of crucial stakeholders
- Provide advice to colleagues
- Provide constructive feedback & solutions to the Area Manager
- Engage in events and presentations on behalf of the Coordination Team
- Qualification in Community Services or Mental Health (or related field)
- Minimum 1 year experience as a Support Coordinator
- Comprehensive understanding of the NDIS and mainstream services
- Ability to meet KPI’s and billable hour targets
- Current Drivers Licence
- Valid QLD and NSW Working with Children Check
- Valid NDIS Worker Check (or willing to apply)
- Demonstrated ability to create and implement person centred services
- Well-developed communication (written and verbal), interpersonal, problem solving, client database management and time management skills
- Experience in networking and positive stakeholder engagement
- Demonstrated ability working with continuous quality improvement processes/outcomes
- Team oriented with an open and collaborative work style
