Reporting to the Indigenous Mental Health Team Leader, the purpose of this role is to provide care coordination and clinical support to Aboriginal & Torres Strait Islander people with mild to complex Mental Health needs. This includes assisting in accessing primary Mental Health Care services, as well as enabling best practice care to be delivered by GPs and other health professionals.
The successful applicant will work closely with numerous stakeholders, including other members of the HealthWISE team, community services agencies, allied health providers and particularly with General Practices in working toward the delivery of a holistic Health Care approach to Aboriginal & Torres Strait Islander community members.

Key Responsibilities
- Promote cultural awareness and competence within the HealthWISE team and in interactions with consumers, stakeholders and communities.
- Promote the safety, wellbeing and empowerment of children.
- Comply with HealthWISE policy and procedures and Code of Conduct as well as all relevant legislation and regulatory standards.
- Actively contribute to the growth and financial sustainability of HealthWISE through identifying new business opportunities, efficiencies, cost savings and innovative solutions.
- Demonstrate understanding and knowledge of /or experience in delivering services via Telehealth as related to this role
- Work closely with the IMH Senior Care Coordinator to provide high level program coordination skills in the planning, development, implementation, promotion, monitoring, evaluation and reporting of services and activities.
- Support the Indigenous Mental Health Team Leader to ensure all contract deliverables within the scope of the position are met and reported within the prescribed timeframe and within the approved budget.
- Enhance the physical and Mental Health of Aboriginal & Torres Strait Islander people living with mild to complex Mental Health conditions who are living in the community, by helping to facilitate management in a way that will result in optimal health outcomes for that individual;
- Improve communication, collaboration, networks and referral pathways between general practice, government and non-government health services and other relevant services which aim to provide an integrated system of care
- Promote HealthWISE IMH programs and services to the Community
- Source specialised support and consultation where needed, facilitate access to community services and appropriate primary Mental Health Care providers as required;
- Support GPs in providing early intervention and preventative Mental Health services where requested;
- Obtain and record accurate health histories and information to ensure compliance to all organisational policies, procedures and legislative requirements;
- Endeavour to build a comprehensive case history incorporating knowledge, skills and networks of individuals and communities to enable them to take better care of their own health;
- Participate in the development and delivery of Mental Health promotion and education strategies, both individual and group, ensuring feedback from the community is incorporated into planning as well as the goals of the organisation;
- Develop partnerships with other health services and Aboriginal community groups to ensure the education and communication of Mental Health issues are culturally appropriate, easily accessible and understood by the community;
- Effectively collaborate with team members to ensure that all HealthWISE services and activities are operating effectively and efficiently, while maintaining a harmonious team environment
- Maintain and provide data and reports to meet organisational and statutory requirements including supporting recommendations;
- Ensure client and community confidentiality is maintained to build and maintain trust within key stakeholder groups;
- Assist in developing and delivering presentation sessions to ensure all program and clinical staff are knowledgeable and aware of relevant program information;
- Other duties as required within the IMH Care Coordinator skills and experience.

Essential

- Demonstrated understanding of issues affecting Aboriginal and Torres Strait Islander people and their health.
- Current unrestricted and unencumbered Driver’s Licence.
- Willingness to travel and work after hours if required.
- Willingness to consent to a National Police Check.
- Hold a current Working with Children Check (NSW) or Blue Card (QLD) or willingness to obtain.
- Maintain vaccinations as required and provide proof of vaccination if requested.
- A minimum qualification of a Certificate IV in Mental Health services and relevant experience in a Mental Health Care coordination role.
- Demonstrated experience conducting Mental Health assessments
- A demonstrated knowledge and understanding of Primary Mental Health Care
- Demonstrated ability to communicate sensitively and to effectively engage with Aboriginal &
- Torres Strait Islander people and communities in a way that fosters mutual respect and regard.
- Demonstrated ability to work independently, as well as work positively and co-operatively within a team environment as well as within tight time schedules and in accordance with variable workload demands
- Demonstrated ability to lead and support other team members
- Demonstrated experience using a computer, in particular Microsoft Word, Excel and Outlook software packages
- Demonstrated high level written and oral communication skills including facilitating activities with a wide range of individuals and groups
- Demonstrated organisational and high level problem-solving skills as well as the ability, consult, liaise and negotiate with GP’s and other health professionals
Desirable
- Experience working and engaging with regional Aboriginal and Torres Strait Islander communities in a primary Health Care setting.
