About Your Role
- Develop a network to deliver individualised care for people with complex medical and psychosocial conditions.
- Provide a care coordination service across organisational boundaries integrated with general practice, community, acute and social support services.
- Ensure people with chronic/complex conditions receive optimum care and support to remain safely in the community.
- Actively contribute to the Integrated Care Team to ensure equitable access to a flexible, supportive district-wide service.
