We are looking for a Care Coordinator to join our Integrated Care team in Wagga Wagga, NSW. The vacancy is Full Time, 75 hours per fortnight, and will be ongoing employment. Part time hours may also be considered for the right candidate.

What will I do?
- Work with healthcare providers and coordinate support for people with complex health care needs
- Inspire confidence in those who feel vulnerable in accessing the services that can help them
- Assist clients in understanding and accessing the supports available to them
- Network and collaborate with applicable services to reduce unplanned admissions to hospital
- Develop and support the implementation of person-centred health care plans.

What do I need?
- AHPRA registration as a Registered or Enrolled Nurse, or Aboriginal Health Worker qualification
- Experience conducting healthcare assessments, developing care coordination plans, and evaluating outcomes
- Knowledge of the complexities and challenges involved in caring for individuals with chronic care conditions
- Exceptional communication and interpersonal skills with the ability to foster positive working relationships
- A commitment to a person-centred approach working in partnership with individuals and families
- Current drivers licence and willingness to travel for work purposes.
