About the role:
Intensive Case Management Services and Targeted Care Packages are Case Management Services Case contracted to Junction Support Services and funded by the Department of Families Fairness and Housing (DFFH)
Intensive Case Management Services: Intensive Case Management Services (ICMS) have been designed to meet the needs of young people who fit the criteria of 'high-risk youth'; young people who are child protection clients and have multiple and complex behavioural and emotional difficulties requiring long term and substantial support.
ICMS focuses on the provision of intensive support to young people with high needs for whom less 'assertive' Case Management practice either has not previously worked or is considered unlikely to be effective. This proactive approach to Case Management is characterised by a high level of contact with the young person, with an intensity of relationship, and strong participation in decision-making by both the young person and, where appropriate, family members and significant others. The services are multidisciplinary and provide intensive youth outreach programs for young people.
Selection Criteria:
- A relevant tertiary qualification such as a degree in Social Work, and one year's experience in the provision of support to young persons with complex needs and challenging high risk behaviours.
- An understanding of the developmental and therapeutic needs of young people who have experienced trauma, abuse and neglect.
- Knowledge of a range of theoretical perspectives with the ability to apply these theories when working with young person's on statutory orders.
- An ability to work within a Case Management framework, including the ability to adequately assess young people's needs and develop, evaluate, and vary plans in relation to those needs.
- Highly developed communication skills and organisational ability.
