Key Worker - Operations

Your role
As a member of the Operations Team, the Keyworker will be allocated to designated participants. As a
Keyworker, you will be responsible for leading and mentoring a team of Disability Support Workers,
overseeing the program with the Regional Manager to ensure that all programs are undertaken
appropriately maintained, documented and reviewed.
You will be a Keyworker of the Operations team, supported by the Regional Manager.
How you will make an impact
• Be familiar with the individual goals and preferences of the people we support
• Support individuals and the team to understand the implementation of programs of support
including Behaviour Support Plans, Allied Health Plans, Individual Support Plans and house routines
• Recognise all support requirements for the participants we support and feedback to Regional
Manager and broader Care Team
• Be actively involved in the development, implementation, monitoring, review and gathering of
evidence for Behaviour Management Plans and Person-Centered plans using an individualised
planning approach
• Assist to gather evidence as required by the Care Team and NDIS Coordinator for NDIS plan review
purposes
• Assist the participants we support to work towards achieving their dreams and goals and support
them to be included in their communities of choice
• Plan and develop nutritious menus in conjunction with Allied Health recommendations and/or
participants choice
• Monitor participant and house Budgetly accounts or cash ledgers and assist the Regional Manager
through advising on their use and expenditure as well as other financial needs and/or participant
requests
• Assist participants with medication administration and requirements as per policy, procedures and
health professional directions
• Assist with the delivery of activities to enhance the development of capacity and skills across
physical, social, emotional and intellectual domains.
• Provide in-home support with health requirements, personal care and daily living tasks such as
washing, dressing, eating, transport and budgeting in alignment with participant needs and Care
Team support requirements whilst ensuring participant dignity is respected at all times

• Where appropriate attend and coordinate participants Allied Health appointments and health
appointments, update Medication Treatment Sheets and notify internal and external stakeholders
of outcomes as required.
• Ensure all documentation required by One Doorway when supporting a participant is completed
promptly and to a high standard.
• Attend and contribute to participant Care Team meetings
• Support individuals and the team to understand the importance of providing a clean and hygienic
home like environment
• Actively engage in creating a safe work environment through identifying, reporting and when
appropriate mitigating, risk and hazards
• Comply with One Doorway Policies and Procedures as applicable to your role
• Understand participants choice and control, dignity of risk and related policy and procedures
• Other duties as required by the business

What we are looking for
• An entry level qualification or Certificate IV in Disability or similar and a minimum of 2 years’
experience in the position of DSW or Allied Health worker
• Able to cope with the physical demands of the job
• Able to perform domestic duties effectively
• Commitment to the rights of people with disabilities to live dignified lives
• Demonstrated ability to work and accept responsibility either individually or in a team
• Patient, flexible, understanding, supportive and caring nature
• Strong computer, written and verbal communication skills
• Experience working within the disability or health care sectors
• Valid driver’s license
Desirable, however not essential
• Experience working within the disability or health care sectors
• Fully comprehensively insured vehicle

• NDIS Worker Screening Clearance issued in the State of Victoria
• Employee Working with Children Check card issued in the State of Victoria

• In the interests of healthy rostering practices, allocation of on-call time will be spread as evenly as
practicable amongst those required to participate in an on-call roster.
