This is to ensure that access, equity, and inclusivity are integrated into every aspect of our work, fostering a culture that embraces diversity and ensures equal opportunities for all
About Us
We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.
What are PHNs?
PHNs have been established with two key objectives:
  • Increasing the efficiency and effectiveness of services for community members, particularly those at risk of poor health outcomes
  • Improving coordination of care to ensure people receive the right care in the right place, at the right time
A typical week may include:
  • Enhance NWMPHN's role as a culturally competent organisation by leading the development, implementation and monitoring the use of an access and equity framework.
  • In consultation with relevant NWMPHN staff, lead, develop and implement strategies to improve the capacity of general practice and commissioned service providers to deliver culturally appropriate, safe and inclusive services.
  • Work with the Evaluation and Research Team to use data to understand the impact of commissioning on the delivery of care to vulnerable and diverse population groups.
  • Identify, develop, and implement relevant access and equity training programs and education for internal staff.
  • Lead discussions with business areas to identify emerging population health and wellbeing priorities and provide advice on key issues and changing population needs for vulnerable and diverse population groups.
  • Participate in regular reviews of performance and demonstrate a willingness to close learning and capability gaps.
  • Participate in strategic planning and policy development as per specialist knowledge and skills
We are looking for someone who has:
  • Tertiary qualifications in health, social sciences, or a relevant field.
  • Strong understanding of the strengths and challenges faced by different vulnerable populations about their health and well-being.
  • Minimum of 3 years experience working with vulnerable populations in the health service sector, particularly primary care.
  • Skills in contract management, health promotion, and/or community development.
  • Ability to analyse and communicate qualitative and quantitative information effectively.
  • Strong communication skills including use of written, verbal, and presentation skills and project management experience.
What's in it for you?
  • A friendly and supportive professional environment
  • Career progression opportunities
  • Great work-life balance (Hybrid options to work from home and in the office)
Other Benefits:
  • Newly introduced Life Leave. Celebrate cultural events and personal milestones with up to 4 days of paid life leave annually.
  • Generous paid parental leave for primary and secondary carers.
  • Additional paid leave at Christmas and New Year.
  • Join our fun social club and get involved in health and wellbeing activities and events at NWMPHN.
  • Accredited breastfeeding-friendly organisation.
The successful candidate will be required to undertake a National Police Check
NWMPHN is proud to be an equal opportunity employer. We recognise the importance of attracting and retaining talent that mirrors our diverse community. Applicants from Aboriginal or Torres Strait Islander background are strongly encouraged to apply.
Applications for this role will take you to the employer's site.
