About the Role
Ongoing Part time - 26 hours per week
The Leaisure & Lifestyle Coordinator role is responsible for coordinating the Community based activities program designed in partnership with clients attending the groups and various activities.
Programs may be delivered on-site or in community settings and transporting clients to the centre and the activities are an important part of the role. During these activities it is recognised that clients of varying abilities may be participating and varying levels of support may be required to provide an inclusive and welcoming environment. TheCoordinator is responsible for overseeing and directing staff and volunteers during community activities.
About You
Minimum Qualifications:
- Certificate IV in Lifestyle & Health, or equivalent
- Certificate in Food Handling
Key Selection Criteria:
- Knowledge of the wellness and reablement approach to aged care with regard to goal setting and care planning.
- Knowledge of issues relating to frail/aged people with disabilities and their carers
- Understanding of Accreditation Standards, quality systems and continuous improvement models
- Working knowledge of MY Aged Care (MAC), Commonwealth Home Support Program, (CHSP) and Home & Community Care progra, (HACC), Home Care Packages (HCP), National Disability Insurance Scheme (NDIS)
- Demonstrated written and verbal communication skills
- Computer literacy
