Cobram Regional Care is currently seeking a dynamic Lifestyle Coordinator to make a real difference to residents in our care by managing our Leisure and Lifestyle program.
This role is all about enriching the lives of the residents in our care, and would suit experienced Lifestyle Coordinators or Diversional Therapists who want to use their passion and experience to design and implement new programs.
This is a permanent part time position working 8 shifts per fortnight
Key accountabilities:
Overseeing a team of dedicated lifestyle staff, the Lifestyle Coordinator is responsible for:
- The development of lifestyle programs based on individual needs, preferences and goals with the aim to enhance Physical, Intellectual, Emotional, Cultural, and social well being
- The promotion of happiness, wellbeing and quality of life for residents through the development of social and cultural activity programs that are tailored to the needs of residents with a view to improving quality of life.
- Conducting programs in a manner that recognises the autonomy and independence of each resident.
- Encouraging positive interaction, improved self-esteem and a sense of achievement in residents by assisting them to participate fully in their chosen activities.
- Preparing comprehensive and accurate resident social and cultural assessments of residents
- Ensuring complete and timely documentation of resident participation and engagement
- Managing and coaching members of the lifestyle team to deliver engaging and meaningful activities
- Supporting the orientation and training of new team members and volunteers
- Developing and maintaining positive relationships with residents, families and volunteers
- Facilitating community engagement opportunities
Essential
- Certificate IV Leisure & Lifestyle or other relevant qualification.
- Experience in the planning, implementation and evaluation of activities and programs for frail people with cognitive impairment and other disabilities.
- Ability to document assessments, care plans and progress notes to meet legislative, funding and quality improvement requirements.
- Ability to work and communicate with people of all ages and cognitive ability and to obtain their trust and co-operation.
- Ability to work as part of a multi-disciplinary team to provide holistic care.
