About the role
An exciting opportunity for a Lifestyle Coordinator has become available at Presbyterian Aged Care, Thornleigh.
As the Lifestyle Coordinator you will be responsible for ensuring the consumer’s leisure needs are met. Your responsibilities will include carrying out assessments and developing and carrying out a lifestyle program.
Key responsibilities:
Carry out a high quality and engaging recreational program which encourage personal development and increases life satisfaction, including group, small group and individual activities
Apply your enthusiastic and empathetic nature to further enhance the lives of our residents.
Reporting to the on-site Local Home Care Manager, you will be responsible for ensuring the consumer’s leisure needs are met and carrying out assessments
Facilitate consumer involvement and engagement in the planning of programs
Ability to create a holistic recreational program which strongly apply the values of person-centred care
Continuously assess the recreational needs of consumers.
Actively monitor and record changes in consumes care plans.
Service operates Monday to Friday, business hours. Role is permanent full time, 5 days per week working from 09.00 to 17.00 at our Thornleigh facility
About You
By nature you are compassionate and have a commitment for improving the lives of others. You have relevant qualifications: Diploma or Cert IV in Leisure / Diversional Therapy and previous experience working as a Leisure and Lifestyle Coordinator in an aged care environment.
Essential selection criteria also includes:
Understands the importance of recreation in improving resident functioning
Experience in carrying out client assessment
Experience in dementia and memory support care
Good computer skills.
Excellent communication skills.
An exceptional understanding of person-centred care.
Excellent administrative and documentation skills.
Experience with building and facilitating recreational programs in aged care
Able to take initiative and be creative, with a ‘think outside the box’ attitude.
Driving licence
Current First Aid Certificate desirable
