Your new team:
CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. This role is a unique opportunity to have a real impact in meeting the needs of one of our community’s most complex and vulnerable cohorts. You will form solid working-relationships within the pre-existing community teams, and foster strong therapeutic relationships with our clients & their carers.
We support rehabilitation goals through the use of best evidence-based frameworks and assertive case management.
Your contribution:
- Provide one to one peer support to consumers using personal Lived Experience to provide a model of hope and recovery, including direct and over the phone support.
- Assist CCS consumers with recovery and wellness planning to help gain a sense of control and empowerment over their lives by building on strengths and resources.
- In collaboration, contribute to the development and review of consumers discharge plans to address needs that impact on discharge into the community.
- Contribute your unique range of personal skills and knowledge to the consumer group during groups and activities/appointments held during usual operating hours
