They will ensure consistent, efficient and effective day to day running of the TACP program to residents of WSLHD.

Registration in line with Award requirements and knowledge of the Transitional Aged Care Package (TACP) program, and other relevant Aged Care Programs and initiatives.
- Demonstrated skills and knowledge in the comprehensive assessment and management of older people with complex care needs.
- Supervisory or management experience in rehabilitation or Aged Care services. Ability to manage and support a multidisciplinary team, planning and coordinating work, coaching and developing staff, managing performance, including the ability to achieve performance targets, reporting deadlines and other key performance indicators
- Demonstrated high-level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases.
- Demonstrated leadership skills including the ability to negotiate and manage conflict.
- Efficient time management and organisational skills
- Current unrestricted NSW Drivers licence.
- Demonstrated commitment to Quality Improvement and ongoing professional development.
