This contract role is based in a highly sought after location due to stunning scenery, an incredible community and life changing experiences.
As a key member of this Mental Health service, you will be responsible for providing expert care and support to individuals in the community. Collaborating with a multidisciplinary team comprising three adult community case managers and an Indigenous health worker, you will undertake crucial duties including intake assessments, triage evaluations, and comprehensive case management.
The successful candidate will be expected to manage approximately 15 cases, ensuring the delivery of high-quality care tailored to the unique needs of each individual.
The Contract: ASAP - up to end of May with possible extension.
