Forensicare is the statutory agency responsible for the provision of adult forensic mental health services across Victoria.

We address the needs of people experiencing mental health issues, the mental health sector, the justice sector and the general community. These services are delivered through inpatient services at Thomas Embling Hospital, community services through the Community Forensic Mental Health Service and prison-based services at several locations. This position will be based at Dame Phyllis Frost Centre and may be required to work across other Forensicare sites from time to time.

Forensicare provides specialist mental health services at Dame Phyllis Frost Centre in the Marrmak Unit, a 20-bed residential program for women prisoners with an intensive outreach program and a therapeutic day program. There is a strong focus on providing a healing environment not only for people in prisons, but importantly also one that supports the well-being and safety of all staff. This approach recognises that many women who are imprisoned are also victims of crime and have challenging mental health conditions as a result. The word Marrmak is from the Woiwurrung language of the Wurundjeri people meaning 'safe'.

About the Role:
The Mental Health Transition Support Clinical Coordinator role:

Holds the transition portfolio at local prison site and will be the first point of contact for transition referrals at Dame Phyllis Frost Centre
Will assist The MHTS Lead to provide clinical leadership within the Forensicare model of care to support transition of care into, across, between, and out of Prison Services
Work closely alongside existing clinical coordinators across Forensic Mental Health Programs to support and have oversight of local transition activity and pathways for MHTSS consumers

Duties:
Responsible for triaging referrals based on identified criteria and liaise with the treating team to ensure people requiring AMHWS referral and support on release are identified and receive appropriate transition supports.

Contribute to the development and maintenance of MHTS systems to monitor wait lists, prioritise activities and ongoing development/evaluation of the service.
Use this system to support operational and clinical activities within existing clinical review processes at local sites.
Support coordination of MHTS consumer case conferences and care team meetings as required.

About You:
Approved qualification in nursing, social work, psychology or occupational therapy and current registration with AHPRA where relevant. Social Workers must be eligible for membership with the AASW.
A minimum of 5 years post qualification experience within mental health, preferably recent experience in community mental health setting or a demonstration of significant and extensive experience working within a secondary/tertiary mental health service.
Thorough understanding and demonstrated commitment to mental health consumer Recovery, including the modelling and leading of Recovery focused initiatives.
Commitment to and demonstrated ability in working collaboratively and effectively with a broad range of internal and external stakeholders.

What you need:
Nursing

Postgraduate qualification in Mental Health Nursing or equivalent.
Registration with the Australian Health Practitioner Regulations Agency (AHPRA).
The successful candidate will be required to register their qualifications on eCredential

Occupational Therapy

Approved degree in Occupational Therapy recognised for membership of Occupational Therapy Australia – Australian Association of Occupational Therapists.
Registration with the Australian Health Practitioner Regulations Agency (AHPRA).
The successful candidate will be required to register their qualifications on eCredential.

Psychology

Masters or Doctorate degree in Clinical Psychology from an Australian Psychology Accreditation Council (APAC) accredited course or equivalent, or a Doctorate degree in Forensic Psychology from an APS accredited course or equivalent.
Registration with the Australian Health Practitioner Regulations Agency (AHPRA).
The successful candidate will be required to register their qualifications on eCredential.

Social Work

Approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers.

COVID19 & Influenza Vaccination Evidence

Under the Victorian Public Health orders, all employees working within the Public Health system must be fully vaccinated against COVID-19 and Influenza and are required to provide acceptable evidence of their vaccination status. The current orders require evidence of three doses for COVID-19.
