Job description

Your LINC is a multi-agency, two-tier Mental Health outreach service delivered in partnership by Centacare, KWY Aboriginal Corporation and Aspire Recovery Connection (ARC) a 100% Lived experience organisation. Our service offers holistic, flexible and culturally responsive psycho-social support for individuals experiencing Mental Health challenges and distress across the Adelaide Metro, Barossa, Hills, Fleurieu and Limestone Coast. It is delivered in the Limestone Coast Local Health Network region.

The Role

Mental Health Workers work with people, and their families/carers where appropriate providing person-centred psychosocial support including goal setting, exit planning, and application and transition to NDIS services.

Tier 1 (6 weeks) - Intensive Crisis Support – provision of flexible high intensity case management support. The key outcomes are reduced levels of distress/increased wellbeing via addressing immediate stressors and crises, and supported transition and exit with step-down provision and warm referrals to services as appropriate.

Tier 2 (4 – 12 month) – Non-acute Psychosocial Support – provision of a moderate intensity case management approach to support people to build independence, connect with natural and community supports, and improve people’s quality of life over a period of 4 months. Tier 2 clients who are likely to be eligible for the NDIS will be supported to test their eligibility, apply for access, and supported through planning and plan implementation to successfully transition out to NDIS services. An extension may be applied for up to a total of 12 months for NDIS applications and exceptional circumstances. The key outcomes are wellbeing via participation in recovery oriented and strengths-based social/recreational/vocational activities, and supported transition and exit to NDIS and services as appropriate.

This position is funded on a fixed-term opportunity until 30 June 2027.

What you need to bring

Essential

  • Certificate 4 level qualification in Mental Health (non-clinical) or equivalent qualification.
  • Experience and expertise in working directly with people with Mental Health issues, complex needs, and with their families and carers.
  • Proven ability to work autonomously and as a member of a team.
  • Demonstrated ability to plan and prioritise to meet client service delivery requirements.
  • Demonstrated experience in documenting client case notes and working with electronic data bases.
  • Demonstrated understanding of available community services, networks and supports.
  • Demonstrated understanding of the NDIS eligibility, application, and transition processes.
  • Great interpersonal and listening skills to build strong relationships with key stakeholders.
  • Excellent customer service skills.
  • Demonstrated experience working with the recovery framework and taking a trauma informed approach to practice.
  • Ability to make sound decisions under pressure and de-escalate crises.
  • Valid Department of Human Services (DHS) Working with Children Check.

Desirable

  • Degree in relevant Human Services discipline.
  • Experience using Penelope electronic client management system.

Special Conditions

  • Flexibility is essential in this role - some work outside of standard hours may be required.
  • Willingness to take part in Centacare’s fleet car program or grey fleet your personal vehicle.
