About the Role:
An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as a Social Worker within the Mental Health Division.
The Acute Stream Social Worker works within the multidisciplinary team and provides specific expertise to consumers experiencing deteriorating mental state. Key aspects of the role include emergency assessments, short term intervention, liaison with inpatient and community services, treatment planning, crisis intervention and safety planning. The Acute Stream Social Worker utilises a recovery oriented and person-centred approach to deliver care, working within a team-based style of service provision. The Acute Stream Social Worker participates in weekday clinical reviews to enable responsive care, promote recovery and service flow. The Acute Stream Social also contributes to the regular review of team practices and supports the development of new initiatives.
The Social Worker (AHP2) is accountable to the Team Manager/ Team Leader of the NALHN Unit for the provision assessment and treatment services for consumers of NALHN Community Mental Health Services, and which aim to optimise Health Outcomes.
The incumbent will rotate through a series of clinical services.
The Social Worker applies clinical experience, increasing clinical knowledge and professional competence to plan, implement and evaluate comprehensive and integrated services to the needs of clients of NALHN.
The Social Worker (AHP2) may provide direction and support to less experienced staff, Allied Health Assistants and Students.
The Social Worker works collaboratively with the multi-disciplinary team and liaises directly with Medical, Nursing Staff of NALHN and with other Community Agencies.
To be eligible for this position, you must have an Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
