Our beautiful Greenway Views village is currently looking for a Move-In Registered Nurse to join our team.
Providing pre-entry clinical assessment, move-in support and care planning the Move-In RN focuses on care delivery and care evaluation prior to residents moving into Greenway Views. The Move-In RN will support resident transition of both respite and permanent residents into the village through an integrated and coordinated care and service approach to care planning and social wellbeing.
This is a permanent full-time position from Monday to Friday, 8:30am to 5pm.
The Move-In RN provides:
- Timely and effective clinical assessment of residents wishing to move into Greenway Views under either our respite or permanency arrangement.
- Access residents with care and support requirements to determine their care needs and plan service delivery accordingly.
- Identify existing care arrangements and arrange for transition to Greenway Views Village.
- Liaison with the resident and their families about accessing externally funded care and support.
- Support residents and their families to navigate care planning options when care needs increase.
- Ongoing clinical review and case management of respite care recipients at LDK’s predetermined scheduled intervals
- Supported the transition from respite to permanency through stakeholder engagement ensuring positive resident outcomes and clear internal communication.
Retention Bonus
An annual retention bonus of will be paid to all RNs (full time, part time and casual), who remain employed by LDK
- Full time RN annual retention bonus = $3,700
- Part time and Casual RN annual retention bonus is paid pro-rata of $3,700 depending on hours worked.
- An additional annual retention bonus payment of $2,300 is available for RNs who hold post grad qualifications or take on additional training responsibility.
The successful applicant will have:
- AHPRA registration as Registered Nurse
- Ability to provide quality care and deliver it with compassion
- Relevant post graduate qualifications (Desirable)
- Experience in the community care sector
