Join our passionate team at Support Nest as an NDIS Case Manager & Care Coordinator. In this vital role, you will provide comprehensive and compassionate support to individuals with disabilities, empowering them to maximize their independence and enhance their quality of life. Responsibilities:
Assessment and Planning:
- Conduct thorough assessments to understand clients’ needs, goals, and risks, ensuring alignment with NDIS eligibility and guidelines.
- Develop personalized support plans in collaboration with clients and their families, focusing on empowering clients and enhancing their independence.
Coordination of Services:
- Coordinate with various service providers, healthcare professionals, and community resources, ensuring seamless service delivery.
- Regularly review and adapt services to meet clients' evolving needs, maintaining our commitment to flexibility and client-centered care.
Advocacy and Support:
- Advocate for clients’ rights and preferences within the NDIS framework, embodying our values of respect and individual value.
- Offer emotional support and guidance to clients and their support networks, ensuring they are informed, empowered, and heard.
Documentation and Reporting:
- Maintain precise and comprehensive records in line with organisational and NDIS standards.
- Prepare and submit required reports, reflecting our commitment to transparency and accountability.
Education and Training:
- Educate clients and their networks about the NDIS and available services, promoting informed decision-making and self-advocacy.
Service Coordination Duties:
- Skillfully match field staff with clients, aligning services with individual client needs.
- Work closely with field staff to ensure appropriate rostering, and scheduling, and act as their primary contact for any issues or queries.
- Act as the initial contact for clients, Support Coordinators, families, healthcare professionals, support workers, and nominated representatives.
- Develop and nurture customer relationships, leveraging expertise to identify and implement solutions for stakeholders.
- Requirements:
- Bachelor's degree in social work, disability studies, psychology, or a related field, or equivalent experience.
- Proven experience in case management within the disability sector, especially within the NDIS framework.
- Excellent communication, interpersonal, and advocacy skills.
- Strong organizational abilities with a focus on detail and documentation.
- Comprehensive knowledge of NDIS policies, procedures, and legislative requirements.
- Ability to work both collaboratively and independently.
