The Support Worker position is responsible for the delivery of direct supports to participants with complex and multiple needs in accordance with their individually funded plan. Individually funded plans are based on the participant’s goals and aspirations, now and for the future.
It also covers their functional support needs for daily living and participation, the support they need to pursue their goals, and how the participant wants to manage their plan over time.
Support Workers will incorporate a client centred approach to deliver service that supports participants in a way that is tailored to meet their individually identified needs and goals. Support Workers will utilise recovery orientated practice principles in the delivery of direct support aimed at the development of participant capacity and resilience.
You will work with participants and where appropriate within their own community of friends, family, and neighbourhood. You will collaborate with the individual’s service coordinator, with guidance from your line management where applicable, in support of individual recovery goals and NDIS plan delivery. Support Workers will utilise a collaborative team approach within a wider team of Support Workers.
