Nurse Home Visitor

A Registered Nurse or Registered Midwife can apply for this role.

This role requires you to deliver the Australian Family Partnership Program (AFPP) to first time mothers in a home setting. The Nurse Home Visitor works alongside a Family Partnership Worker to deliver the program content, building therapeutic relationships, stakeholder engagement and health promotion. All AFPP staff receive intensive training in their AFPP Home Visit Guidelines, screening tools and other materials to best support parents in improving pregnancy outcomes, improving child health and development, and supporting parents to develop a vision for their own futures.

This strength-based program focuses on 5 client-centred principles:

• Focus on strengths

• Focus on solutions

• Only a small change is necessary

• You are the expert of your life

• Follow your hearts desire

Desirable

• Interest in Aboriginal and Torres Strait Islander Health, midwifery, child health or family centred practice

This position will report to and regularly engage in reflective supervision with the AFPP Nurse Supervisor and will function as an integral part of the AFPP Home Visiting Team.
