The Role
An opportunity has become available for a Nurse Navigator (Older Persons Mental Health) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and coordinating consumer care
- Provide care coordination across the patient journey for a selected cohort of patients using a multi-disciplinary, holistic approach
- Provide a central point of communication, engagement and coordination for all stakeholders
- Establish effective discharge pathways for consumers
About you
To be successful in this position you will require:
- A Bachelor of Nursing or equivalent
- Registration with the Nursing Midwifery Board of Australia
- Credentialed through the College of Mental Health Nurses (or prepared to work towards a credential)
- Driver Licence
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Declaration of serious discipline history must be disclosed
- Criminal History Screening will be required for new employees (GCH will cover cost)

Salary
- Temporary full time (12 months with the possibility of extension)
- Base salary ranging from $132 562 - $143 924 per annum

The Nurse NavigatorOlder Persons Mental Health position provides comprehensive, advanced and person centred specialist Older Persons Mental Health nursing care and is co located with the Older Persons Mental Health Community Service, within the division of Mental Health and Specialist Services.
