ABOUT THE ROLE
Be part of an innovative new model of care for residents of Aged Care Facilities.
Hunter Primary Care’s established Nurse Practitioner Service provides primary care in-reach clinics within residential Aged Care homes in Newcastle, Lake Macquarie and Port Stephens, as well as a new pilot program servicing Tamworth and surrounds.
Due to the expansion of the service, we have several opportunities for self-motivated, experienced, and collaborative Nurse Practitioners to join our team in either region, providing excellence in the clinical management of older people. These opportunities are offered on a part-time or full-time basis, and relocation support is available on request
Working amongst a team of highly skilled Nurse Practitioners this role includes clinical leadership, assessment, diagnosis, therapeutic management and evaluation, and is involved in ongoing research into the role of Nurse Practitioners in supporting people residing in Aged Care facilities.
SKILLS & EXPERIENCE
- Currently registered as a registered nurse with the Australian Health Professionals Regulation Agency (APHRA) and endorsed by the Nursing and Midwifery Board of Australia to practice as a Nurse Practitioner
- Ability to deliver evidenced based, person-centered care within the Aged Care environment, work autonomously and collaboratively, using advanced and extended clinical skills
- Demonstrated knowledge and understanding of regulatory compliance governing the Aged Care environment
- Highly developed communication skills with a demonstrated ability to manage successful working relationships with residents, families, General Practitioners and the clinical services team.
- Demonstrated effective time and schedule management to ensure residents expectations and health care goals are met
- Evidence of full vaccination against COVID-19
- Current NDIS Worker Screening Check clearance
- Recent or new (employer facilitated) Criminal History Check. Please note that people with criminal records are not automatically barred from applying for this position. Each application will be considered on its merits.
