About the Residential InReach Team:
The Residential In-Reach Team (RIR) is located at the Austin Hospital Campus of Austin Health and is tiered within the Continuing Care Clinical Service Unit. The service is funded via the Department of Health (DoH), Victoria, for the purpose of reviewing people living in residential aged care facilities (RACFs), to provide a timely, person-centred management approach for acute medical conditions.
About the role
- Facilitate a model of care that provides an alternative to ED presentation.
- In collaboration/consultation with GP, Locum Service and Emergency Department, provide acute assessment and short-term treatment of residents in the aged care facility.
- Specifically, this role will provide support for the residential In-Reach and TCP including the acute medical care of patients who currently reside in residential aged care settings, with a strong emphasis on delivering person-centred care, and delineating the goals of treatment in consultation with the person and their medical treatment decision makers.
- In consultation with the multi-disciplinary team, provide additional and specific supports to residents and staff within Austin Health and/or Residential Aged Care Facilities and TCP.
- Practice at an advanced level within the specific scope of practice in the provision of high quality, culturally sensitive resident care in partnership with residents, and their significant others and other members of the multi-disciplinary team.
- Provide an advanced level of clinical knowledge and expertise in the assessment and treatment of Aged Care Residents.
- Support clinical governance of the TCP Residential program working alongside Geriatrician, GP, TCP Clinical Co-Ordinator and Manager.
- Work within DHHS guidelines for “Residential In-Reach Programs”
