Form of workThe Heart Recovery Service (HRS) are looking for a Nurse Practitioner to join their team. The HRS is a multidisciplinary service which sits within the cardiology department at Princess Alexandra Hospital within the Division of Heart, Lung, and Critical Care Services (HLCC). The HRS provides an integrated multidisciplinary case management model of care for patients with complex cardiovascular conditions including heart failure.

About the Role
The Nurse Practitioner - Heart Recovery will provide and coordinate comprehensive care of patients using advanced practice skills in the area of Cardiovascular Nursing, specifically Heart Failure.
The role of the Nurse Practitioner Cardiology will:
• Function in a collaborative model with the healthcare team to provide assessment, diagnosis, treatment and referral as required.
• Provide safe, quality, cost effective care and practice within the Nurse Practitioner's scope of clinical practice, that is consistent with contemporary standards and evidence-based practice.
• Work collaboratively with the health care team and contribute to enhanced patient outcomes by actively leading and supporting existing and emerging nurse-led models of care whilst maintaining flexibility and responsiveness to evolving patient and service needs.
• Provide referrals to appropriate agencies, including primary health settings.
