Your opportunity
This role works collaboratively with the interprofessional team and contributes to enhanced client outcomes by actively leading and supporting existing and emerging Primary and Community Care models of care whilst maintaining flexibility and responsiveness to the needs of the health service and client populations.
This position is required to travel and work across the South West Hospital and Health Service.
