The company's committed mobile allied health team includes Occupational Therapists, Physiotherapists, Exercise Trainers, Massage Therapists, Nail Care Therapists and in home Nursing. Summit Health Solutions aims to help individuals maintain independence and mobility to carry out activities of daily living as independently as they can.
We are looking for a full-time Occupational Therapist to join us. The Occupational Therapist will be responsible for conducting clinical assessments of clients, preparing reports, conducting equipment trials and communicating directly with clients and their families. We have roles located in Canberra, ACT and South East QLD. You will work with diverse individuals to help them regain their independence and enhance their quality of life.
The candidate in mind will be expected to perform the following duties that include but are not limited to:
- Able to assess functional limitations of people resulting from illnesses and disabilities, and provide therapy to perform their daily activities and occupations
- Able to plan and direct programs using vocational, recreational, remedial, social, and educational activities on an individual and group basis
- Using various clinical observations and standardised tests to assess client's emotional, psychological, developmental, and physical capabilities and assisting during Mental Health, Early Intervention, Activities of Daily Living, and Exercise.
- Assessing client's functional potential in their home and leisure area whilst recommending environmental adaptations to maximise their performance and ability
- Providing advice on various adaptive equipment, such as wheelchairs and walking aids, to assist clients to overcome their functional limitations
- Providing professional expertise and advise to family members and carers about adapting clients' home and leisure environments
- Evaluate progress and outcomes, by using person centred tools and functional observations and ability to maintain clear records, follow organisational policies and procedures within an expected timeframe
- Managing a caseload of clients, writing of reports and programs to achieve client centred goals and outcomes
- Experience or willingness to work within aged care, rehabilitation, and community service.
- Skilled in conducting comprehensive clinical assessments and developing individualized plans that address patients' needs and lifestyle goals.
- Maintain accurate and up-to-date client records, including assessments, progress notes, and treatment plans.
- Evidence-based decision-making, problem-solving, and critical thinking skills
- Effective communication and interpersonal skills for interacting with clients and their families, colleagues, and stakeholders.
- Bachelor or Master of Occupational Therapy
- Current registration with AHPRA (Australian Health Practitioner Regulation Agency)
- National Police History Check
- Working with vulnerable people card (aged care sector check) or Willing to obtain.
- COVID-19 vaccination plus booster, current flu vaccination
