Hesse Rural Health is a key integrated rural health care provider located within the three local government areas of Surf Coast, Colac Otway and Golden Plains. The organisation provides a broad range of services which include: Aged Residential including Dementia Specific Care, Sub-Acute Hospital, Urgent Care, Community Health, Health Promotion, District Nursing, Allied Health, Palliative Care, Home Support Program, Home Care Packages, Social Support Program Groups and Facilitated Play Groups.
This position is a diverse role comprising OT specific assessments, interventions and planning predominantly in Sub-Acute and Aged Care frameworks to increase people’s function and independence in all aspects of their lives. The position of Occupational Therapist is to provide a high quality, effective and efficient service to clients accomplished by working as a member of the integrated health care team with a focus on providing person centred, holistic and goal directed care that promotes clients’ wellness and active participation in decisions about their care. The Occupational Therapist works within a multidisciplinary team of nursing, care staff, allied health and allied health assistant staff.
Key Selection Criteria
- Australian Health Practitioner Regulation Agency (AHPRA) registration as an Occupational therapist.
- Demonstrated clinical experience including functional assessments, prescribing equipment and assistive technologies and home modifications, capacity building and improving functional independence.
- Demonstrated knowledge and or experience in integrated health, allied health and primary care with the ability to work collaboratively in an integrated team to achieve agreed deliverables.
- Ability to organise and proactively self-manage workload to meet annual performance targets and service initiatives.
- Proven ability to make rational decisions and sound judgement with a strong commitment to delivering high levels of customer service.
- Willingness to embrace change with the ability to be flexible and adaptable to changing priorities and objectives.
- Well developed interpersonal skills; verbal and written communication skills with the ability to adapt communication style when liaising with people from a diverse range of cultures and backgrounds.
- Strong Computer Literacy Skills (Word, Excel, Outlook, Electronic Health Record).
- Current Victorian driver licence
