Looking for a Palliative Care Specialist to enhance services and outcomes for individuals receiving Palliative Care across the company, including Retirement Living, Residential Services, and Community Health and Care (CHC). This role involves collaboration across various operational units, working closely with the Quality, Risk & Safeguarding (QRS) and Learning and Development (L&D) teams.
This will also develop and implement a comprehensive strategy to elevate the organisation's proficiency in Palliative Care. This is a permanent, part-time role working on Tuesday, Wednesday and Thursday.
Based at Support Office, East Melbourne (with travel to required sites)
Duties:
- Design and implement a Company Model of Palliative Care (BMPC) that improves Quality of Life outcomes
- Develop/update Policies and Procedures as they relate to Palliative and End-of-Life care and ensure they are effectively communicated to all employees
- Work closely with senior management team, Quality, Risk & Safeguarding, and L&D teams to develop and implement a comprehensive strategy to elevate the organisation's proficiency in Palliative Care
- Ensure compliance to regulatory standards and conduct audits of Palliative Care
- Work with the Pastoral Care Team to develop a program, provide tools and evaluate the effectiveness of their input on the palliative and end of life experience
- Develop a brief and communication collaterals to implement Palliative Friendly environments in all Residential homes
- Work with CHC to develop Palliative Care specific packages for people requiring this support in their own homes and within Respite Centres
- Identify services and develop fee-for-service financial modelling for private clients
- Make recommendations for innovative palliative services and work with the Finance team to assess the financially sustainability of such services
- Build customer relationships with external agencies referring Palliative / end of life clients and residents
- Travel is required to sites
We’re looking for someone with:
- Registered Nurse or Allied Health Practitioner (essential)
- Minimum of 5 years’ experience in the delivery of Palliative Care as a practitioner (essential)
- Tertiary qualifications in Palliative Care, or equivalent, with significant experience in Palliative Care (essential)
- Palliative Care Nurse Practitioner (highly desirable)
- Demonstrated senior clinical and management experience within aged care and Palliative Care services
- Knowledge and sound understanding of the quality standards in aged care and their application to Palliative Care and end of life
- Experience in developing frameworks that lead to quality of life outcomes for people with life-limiting illnesses and nearing end of life
- Project management with demonstrated ability to critically evaluate success and impact of programs
- Experience with cultural change and change management
- $74 hourly rate
