The Palliative CareTransitional Nurse Practitioner/Nurse Practitioner (TNP/NP) provides a readily available access to high level clinical expertise and support to community Palliative Care clients, residing across the Western Sydney Local Health District (WSLHD) community. The TNP/NP is an expert clinician who manages and coordinates episodes of care as the primary provider or as part of the multidisciplinary team. The TNP/NP functions as an expert nurse clinician with extensive experience in Palliative Care Nursing who is authorised to function autonomously and collaboratively in an advanced and extended role which is not able to be performed by a RN or Clinical Nurse Specialist (CNS).The TNP/NP provides evidence based autonomous clinical care for Palliative Care clients within approved, established guidelines for advanced nursing practice and understands their accountability and works with wider care team to secure the most appropriate care for the clients. In addition, the TNP/NP will have the required knowledge, skills and attributes to utilise latest evidence to inform practice, deliver and coordinate care.How to Apply
