Applications Close: 23 April 2024What you'll be doingThe Transitional Nurse Practitioner (TNP)/ Nurse Practitioner (NP) will provide a high level of evidence-based nursing care within the specialty area of Palliative Care - End of Life nursing.The TNP/NP role will improve access of care and address gaps in existing health care services through the provision of a flexible and innovative model of care.The TNP/NP will provide clinical and professional leadership as an expert in the clinical field and will undertake comprehensive health assessments and collaborate and consult with other members of the health care team to plan, implement, and evaluate care. This integrated approach will support best practice and maximise patient care outcomes.A Nurse Practitioner must have current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) who is endorsed to practice as a Nurse Practitioner by the Nursing and Midwifery Board of Australia.A Transitional Nurse Practitioner must be a Registered Nurse (AHPRA) working towards Nurse Practitioner endorsement with at least 5 years post graduate experience and at least 3 years in the relevant field. The TNP is appointed for a provisional period of up to three (3) years until the applicant meets the NMBA requirements for Nurse Practitioner endorsement.All NSW Health workers are required to have completed an up-to-date course of a COVID-19 vaccine (2 doses) which has been approved or recognised by the Therapeutics Goods Administration (TGA). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an approved medical contraindication certificate certifying the worker cannot have any approved COVID-19 vaccines available in NSW.Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate.   • Demonstrated ability to work independently and in the absence of a Medical Practitioner to provide quality care together with the capacity to work collaboratively as part of a multidisciplinary team.
  • Demonstrated understanding of integrated care and proven ability to work collaboratively within a multidisciplinary framework and mixed clinical environment and evidence of the ability to influence organisational and cultural change.
  • Demonstrated advanced clinical skills, competence and leadership in the assessment, treatment, and management of patients with Palliative Care and End of Life care needs.
  • Demonstrated knowledge and understanding of current issues impacting on the provision of Palliative Care services in regional and rural areas and incorporation of same in the development of strategic and clinical service plans.
  • Demonstrated high level advanced communication and interpersonal skills (including the use of coaching) across a variety of mediums and proven ability to write for a variety of documents, including Executive reports.
  • Proven information technology skills with specific experience in the use of word processing, database, and spreadsheet software.
  • Evidence of participation in own professional development, a commitment to maintaining competence and enhancement of skills, willingness to apply and share professional expertise as a resource person and mentor others within the field of practice.
