We specialise in in social prescribing, linking people with physical and mental health care needs to resources, with non-clinical sources of social support within their community. For us health is about quality of life, including creating opportunities to address any social determinants of health.
We are looking for qualified Social Workers, and allied health professionals who would love to connect people into community, help people find services they needs, and improve the ability of community members to access services and supports.
In this role you will help enable clients’ self-advocacy and self-management, contribute to improved health literacy and equitable access to services, and enhance a supportive environment allowing clients, their family, friends and carers and their network of health service providers to address the social factors that contribute to rising chronic illness, mental health issues, isolation and loneliness.
