About the role
An exciting new opportunity exists for a Registered Nurse who has experience in facilitating safe and effective discharge planning for patients in our acute Malvern setting.
- The Patient Flow Coordinators collaborate closely with our Hospital Coordinators and Clinical Operations team, focusing on enhancing the progression of care and streamlining Patient Flow throughout the hospital.
- Reporting to the Nurse Director of Clinical Operations, the Patient Flow Coordinators will prioritise timely and safe discharges for patients with approaching discharge dates and with a known destination.
- These roles will work across the middle of the day into the evening and will require some weekend coverage.
- Patient Flow coordinators will be required to provide support to all specialities (medical/surgical/emergency) across the Malvern campus.
About you
You will be required to have
- Current registration with the Australian Health Practitioner Regulation Agency
- High level of clinical competence (would suit NUM/ANUM/Hospital Coordinator)
- Demonstrated understanding of safe and timely discharge principles
- Ability to work collaboratively. Demonstrating effective negotiation, consultation, and facilitation sills.
- Flexible and adaptable work style to support different wards and units.
- Well-developed communication skills - Demonstrating the ability to provide quality customer service to various stakeholders in a large and complex organisation.
