About the Position:
Lumia Care are looking for an experienced Physiotherapist who thrives in working within a multidisciplinary team and will be committed to upholding and promoting the organisation's core values. You will bring a strong passion for client Care and a high standard of customer-focused service.
Lumia Care have a number of roles across Greater Sydney, the Central Coast and Newcastle. This role will require you to travel and attend to clients homes and shared homes within a community and home based setting.
Key responsibilities:
- Community based role with a strong focus on enhancing the independence of our clients on short term programs.
- You will have the ability to build trust and rapport with all your clients
- Carry out an intensive assessment of their needs and create personal care plans so they can achieve the goals you have both set.
- You will organise other resources to assist with the care plans if they are a requirement
- You will be up to date to with funding streams, access to equipment, services and community groups to also share with your clients.
- The ability and understanding that every client is different, each will have their own challenges and personal goals.
- Collaborate with both internal and external health professionals and services.
- Liaise with families/care providers by providing ongoing support, care and education
- The ability to work independently and good time management skills
- Plan and carry out relevant training/handovers to staff including Therapy Assistants

About you
- Full AHPRA Registration with no restrictions
- Demonstrated experience in client work in a community, hospital or rehabilitation setting
- Sound knowledge of assessments and treatments
- Excellent client centred focus along with effective communication/problem solving skills
- Knowledge and understanding of funding and services
