MARS Healthcare are seeking an experienced Physiotherapist to join our trusted client's dynamic multidisciplinary team! Servicing a patient caseload, based South of the River in Perth WA. This role offers a unique opportunity to contribute to community services, homecare packages and 8-week goal-led programs while collaborating monthly with a passionate team who are all dedicated to a shared goal!
Responsibilities:
- Manage a caseload within the Perth southern metro area.
- Conduct homecare visits and contribute to 8-week goal-led homecare programs.
- Participate in team meetings and collaborate with other AH disciplines.
- Ensure meticulous documentation, meeting legislative and accreditation standards, and supporting the ACFI process by collating information from progress notes.
- Conduct assessments on residents' physical and mobility functions, addressing falls risk and ensuring effective transfers.
- Collaborate with residents/representatives, medical requirements, and input from health professionals to plan, implement, evaluate, and review treatment and mobility programs in alignment with organisational policies.
Qualifications and Skills:
- Bachelor Qualification in Physiotherapy
- Demonstrated experience as a physiotherapist.
- Current AHPRA registration
- Knowledge of Aged Care standards
- Effective use a healthcare IT systems.
- Evidence of Covid and Influenza Vaccination (preferable)
- National Police Clearance (obtained within the last 6 months)
- Experience in homecare and family community settings preferred.
- Familiarity with Aged care standards.
- Collaborative mindset, willing to share skills and provide guidance.
