The Grade 2 Physiotherapist, through their clinical expertise, experience and skill, will be responsibile for delivering excellent, evidence based patient management within the home setting.This position will be appointed to the Western Health at Home teams, intially providing assessment and treatment to patients in the Rapid Allied Health (RAH) service. There will be an opportunity to work in other teams in the Western Health at Home Division inlcuding in GEM@Home, Community Based Transition Care Program (CB TCP), Community Based Rehabilitation (CBR) as required.This position is available on a permanent full- or part-time basis.The Western Health at Home team is looking for candidates that:
- Possess strong interpersonal and clinical skills
- Work collaboratively within the inter-disciplinary team
- Demonstrate critical thinking in practice
To be Successful in the Role, you must have:
- Appropriate recognised tertiary qualification and Registration with the Australian Health Practitioner Regulation Agency (AHPRA) as eligible to practice as a Physiotherapist
- Experience/qualification working with older adults, community based physiotherapy and/or a willingness to learn
- Highly developed interpersonal, written and electronic communication skills
- Excellent organisational and time management skills
- Commitment to continuing professional development
- A current Victorian drivers licence
