AddressThis role provides home visits and centre based appointments across the greater Toowoomba region. You will be providing integrated therapy with our multidisciplinary Allied Health and Care teams, supporting aged-care clients to enhance their mobility and movement and promote their independence and wellbeing. Your part in our Blue Care team
- Permanent Part Time community service delivery role
- Expressions of interest sought for minimum 3 weekdays/week .
- Diagnosing, assessing and implementing care plans for individuals and groups
- Prescription and selection of appropriate adaptive equipment and modification
- Education and training on aspects of therapy for the clients and carers
- Goal setting activities, as well discharge and resolution planning
- Liaising with healthcare professionals, such as GPs, nurses and client care managers to ensure care plans are formulated and monitored.
What makes a difference for us?
- Tertiary Qualifications and current AHPRA registration
- Current Australian Drivers Licence and a road worthy vehicle with comprehensive insurance
- Relevant experience within Community Services or Rehabilitative Care (or equivalent)
- Experience in telehealth service delivery would be highly regarded
- Exceptional communication skills with the ability to build great relationships
- Effective time-management and proven clinical problem-solving skills
- Confidence to satisfy the relevant probity checks required by legislation or policies
