As a Physiotherapist with Lifecare Homecare you will be providing high-quality treatment to our patients to ensure the provision of optimal health outcomes.
Lifecare Homecare provide allied health services to individuals in their home, with a focus on aged and disability care therefore we are seeking a Physiotherapist with an interest in working with the older adult and/or disability demographic. You will work across a variety of case presentations and clients across a defined region, Mandurah, providing care for our elderly community in the comfort of their home with our home visiting service.
In this role you will be:
- Delivering quality physiotherapy services tailored to the individual needs of the client and using a person-centred approach
- Conducting physiotherapy assessment and management of patients in a community setting in their own home
- Using clinical reasoning, to create and implement appropriate treatment plans, which target the goals of the patient and with a particular focus on achieving functional goals and supporting independent living
- Liaising with other Allied Health Professionals, GP’s, Surgeon’s, Sports Physician’s and other Health Care Professionals, as required, within and outside the team, to help provide holistic care to patients
- Preparing initial reports, progress reports, final reports or treatment management plans, which meets a standard that complies with the various funding bodies, such as NDIS, DVA, Medicare and HCP Providers
