About the role:
The overall purpose of the Practitioner is to provide case management, care and support to young people living in our accommodation or in the community. The care and support provided will enhance opportunities for the young person to participate in appropriate activities to support development community inclusion, positive relations with family of origin and to contribute to their accommodation stability.
This is a Permanent Full-Time position working 76 hours a fortnight based at our Beenleigh, Insync Youth service.
This role provides support to young people living in our transitional housing, as well as providing centre based and mobile support to young people in the community. You will be focused on encouraging participation in activities, supporting community inclusion and contributing to their accommodation stability. You will value differences, self-determination, social justice and provide support to these young people on their journey towards independence.
Responsibilities:
- Implement Anglicare’s Mental Health and Well Being framework effectively.
- Collaborate with individuals and families, advocating for their needs and accessing relevant resources.
- Contribute to strategic planning reviews and project development.
- Aid in creating promotional materials and provide supervision as required.
- Maintain thorough documentation and assist in policy development.
- Keep external resources updated and assist with ongoing administrative tasks.
- Foster positive team dynamics and actively participate in meetings.
- Offer support to team members and contribute to process improvements through shared insights and professional development.
Required Qualifications and Credentials:
- Degree in Health, Human Services, Behavioural Science, Social Work, Psychology or similar (this is an essential requirement of the role)
- Current Blue Card
- Current Queensland Driver Licence
