Job description

The Role

The Program Officer, Palliative Care is responsible for increasing awareness of and facilitating access to safe and high-quality palliative and end-of-life care at home. By linking education and training resources through palliative care projects, this role will collaborate with all levels of health networks and communities in the Western Queensland region.

The role is responsible for the following key functions within WQPHN:

  • Contribute to the collection of quantitative and qualitative data and outcome measurement including baseline data. Contribute to and provide data and information for the national evaluation, improving coordination between health services.
  • Support the Coordinator Palliative Care with Hospital and Health Services and non-government providers for the provision of home-based palliative care.
  • Support the creation of outcomes that enhance the standard of palliative care in Western Queensland. This will include linking and drawing upon education and training already existing in the region and available via National Palliative Care Projects. Local and state government networks will be included to ensure that this project maps services in the WQPHN region.
  • Collaborate with the Strategy, Planning & Performance team to ensure all activities support WQPHN’s commissioning model, data requirements, and the Activity Work Plan (AWP) to collaboratively build capacity and meet the identified complex needs of the local communities.
  • Contribute to service integration initiatives such as improving access for target groups, Closing the Gap, integrated care models, case management of complex chronic conditions, prevention and early intervention strategies.
  • Contribute to staff training and support professional collaborations or forums and quality improvement networks.
  • Contribute to the promotion and integration of Palliative Care into the wider Western Queensland health system by working collaboratively with other health and service providers.
  • Generally support the Coordinator and Senior Manager and other staff with efficient management of records, preparation and editing of submissions and reports, distribution of materials and correspondence.
  • Assist with travel logistics for meetings and events, including room/venue booking and catering.
  • Work collaboratively with all staff, providing administrative support as required.
  • Provide a regular update on key achievements, challenges or potential risks and issues.
  • Undertake other such duties and training as may reasonably be required and which are consistent with the level of responsibility of this position.

The Right Person

The successful candidate will possess: 

  • Demonstrated understanding of palliative care concepts and practice, and multi-disciplinary education and training methods would be highly regarded. 
  • Experience as a Program or Project Officer in community engagement with a focus on data collection, collation and primary health care client record keeping and reporting systems or similar is essential.
  • High level understanding of the health, social and emotional well-being needs of regional, remote and rural communities including Aboriginal and Torres Strait Islander Peoples with the ability to engage a range of communities and community leaders.
  • Demonstrated ability to prepare clear and concise business communication documents including reports, budgets, meeting minutes, briefing notes, letters and staff correspondence.
  • Ability to work with minimal supervision, manage conflicting deadlines and demonstrate effective problem- solving skills.
  • Proficiency in Microsoft Office (including SharePoint) and other office productivity tools, with aptitude to learn new software and systems.
  • Hold a current driver’s licence, unrestricted for work purposes.

Our Working Environment

Ideally, this critical role will be based within the WQPHN region in Winton, Longreach, Mount Isa or Roma; however, applicants are welcome to apply from across the state. Regular travel will be required across the WQPHN patch to successfully perform this role, and hybrid work-from-home arrangements also apply.

The remuneration package for this role includes attractive tax-exempt salary packaging options.
